Work health and safety risk assessment and incident register — every PCBU (person conducting a business or undertaking) has a duty to manage risks so far as reasonably practicable under the model Work Health and Safety laws (⚠️ Victoria instead uses the Occupational Health and Safety Act 2004, with similar duties), including psychosocial risks.

WHS RISK ASSESSMENT AND INCIDENT REGISTER
Business (PCBU): Business name (PCBU) State/territory: State / territory Assessment date: Assessment date
PART A — RISK ASSESSMENT
This assessment is carried out to meet the primary duty of a person conducting a business or undertaking (PCBU) to manage risks to health and safety so far as is reasonably practicable, under the model Work Health and Safety Act and Regulations adopted by most states/territories. ⚠️ Victoria applies the Occupational Health and Safety Act 2004 instead of the model WHS Act — the duties are broadly equivalent but the underlying legislation is different.
1. HAZARDS IDENTIFIED 1.1. Hazards identified 1.2. This assessment specifically considers psychosocial risks (e.g. aggression from customers, workload, bullying), which are an explicit duty under the model WHS Regulations.
2. WHO IS AT RISK 2.1. Who is at risk
3. CURRENT CONTROL MEASURES 3.1. Current control measures
4. RISK RATING 4.1. Risk rating (after current controls)
5. FURTHER CONTROLS 5.1. Further controls (hierarchy of controls) 5.2. Responsible person and due date: Responsible person & due date for further action
6. REVIEW 6.1. This assessment must be reviewed regularly and whenever circumstances change. Next review: Next review date.
PART B — INCIDENT REGISTER
7. INCIDENT RECORDING AND NOTIFIABLE INCIDENTS 7.1. Incident register & notifiable incident procedure 7.2. A "notifiable incident" (death, serious injury or illness, or dangerous incident) must be reported to the state/territory WHS regulator immediately, and the incident site preserved until an inspector arrives or directs otherwise. ⚠️ A register of injuries is separately required under workers compensation legislation in some states (e.g. NSW).
8. WORKERS COMPENSATION 8.1. Workers compensation insurance policy on file. Workers compensation insurance is compulsory in every state/territory (e.g. icare in NSW, WorkCover in QLD, WorkSafe in VIC) — confirm the current policy is in force.

— — — ⚠️ This is a boilerplate template, not legal advice. Australian workplace, consumer-protection, food safety, and work health & safety law combines uniform federal rules (Fair Work Act 2009, the National Employment Standards, the applicable Modern Award, the Australian Consumer Law) with state- and territory-specific rules that this template flags with ⚠️ but does not resolve for you — have it reviewed and adapted by a lawyer before use, check the current Fair Work Ombudsman resources at fairwork.gov.au (including free official templates and the P.A.C.T. pay calculator), and confirm the Modern Award classification and minimum pay rate in force on the date of use (rates change every 1 July).

Elimination, substitution, engineering controls, administrative controls, PPE — in that order of preference
Register of injuries and a procedure for notifiable incidents (death, serious injury/illness, dangerous incident) — immediate notification to the state WHS regulator is required, and the incident site must be preserved; incident records should be kept for at least 5 years
