1. FAIR WORK INFORMATION STATEMENT (FWIS) 1.1. Issued on: Fair Work Information Statement (FWIS) issued on. Mandatory for every new employee before or as soon as possible after their start date (s 125 Fair Work Act 2009). Use the current version published at fairwork.gov.au — the Fair Work Ombudsman updates it, typically around 1 July each year.
2. CASUAL EMPLOYMENT INFORMATION STATEMENT (CEIS) 2.1. Issued on (casual employees only): Casual Employment Information Statement (CEIS) issued on. Must also be reissued periodically — see the separate CEIS re-issue reminder record.
3. TAX FILE NUMBER (TFN) DECLARATION 3.1. Status: Tax File Number (TFN) declaration status. Date: TFN declaration date. An employee must provide a TFN declaration (or advise they have applied for or declined a TFN) within 28 days of starting, or the Employer must withhold tax at the top marginal rate.
4. SUPERANNUATION 4.1. Status: Superannuation standard choice form status. Date: Super choice / stapled fund request date. If the employee does not choose a fund, the Employer must request their "stapled" super fund details from the ATO before contributing to a default fund. Superannuation Guarantee contributions are payable on ordinary time earnings at the current statutory rate.
5. PAY AND CONTACT DETAILS 5.1. Bank details for pay received: Bank details for pay received. 5.2. Emergency contact: Emergency contact on file.
6. RIGHT TO WORK 6.1. Right to work (VEVO) check. Check date: VEVO check date. Employers can face significant penalties under the Migration Act 1958 for allowing a person without a valid working right to work.
